Workshare Form

Workshare Form, Word and PDF formats

This form will help you to keep track of and receive credit for your workshare hours.  For each volunteer activity, please fill in the date, your activity and time spent.  You will need to have it signed by the director or person running the event or by the office or maintenance manager if applicable.  At the end of the season submit this form to the office no later than the date on the form to receive your credit.  No credit will be issued without a signed form for audit and accounting purposes.  Couples should record both names.  The office will be happy to provide you with a copy for your records once it has been approved by the office manager.  Thank you for helping to keep Solair the wonderful place that we all enjoy so much.
